Yeah the gun in the ground thing has happened in a few games present and in the past. sooo bloody annoying it is too. I've searched an area hearing this clattering and banging going on, only to find the weapon stuck in the scenery.
The other annoying thing is a machine gun sound-loop. Find the gun lying on the ground with a continual firing loop. I've had my side-kick in Skyrim spend an entire 1hr sliding about on it's back following me! Hilarious it was too when she was attacked by wolves, just lying there and fighting them with no attacking animation. Also she went along the bottom of rivers and popped out on the banking.
Far Cry 2 had plenty where my fav was an enemy stuck in the engine compartment funky chicken style, just banging up and down with their feet hanging out.
